Key Responsibilities
• Deliver comprehensive home-based primary care visits tailored to the needs of medically complex patients.
• Utilize telehealth to assess patients as appropriate, integrating findings into personalized care plans.
• Diagnose and manage both acute and chronic medical conditions effectively.
• Collaborate with an interdisciplinary team to develop, implement, and adapt individualized care plans.
• Prescribe medications and order or interpret diagnostic tests within your scope of practice.
• Coordinate care transitions to prevent unnecessary hospitalizations and emergency visits.
• Maintain close communication with physicians, nurses, social workers, and care managers.
• Educate patients and their caregivers on disease management, medications, and preventive care strategies.
• Accurately document all patient encounters in the electronic medical record (EMR) in a timely manner.
• Engage in quality improvement initiatives and adhere to the latest evidence-based guidelines.
Essential Skills
• Expertise in primary care clinical assessment and diagnosis.
• Proficiency in managing chronic diseases such as CHF, COPD, and diabetes.
• Knowledge of geriatric and palliative care principles.
• Experience in home-based and field practice.
• Strong skills in prescribing and medication management.
• Ability to order and interpret lab tests and diagnostic imaging.
• Efficient in electronic medical record (EMR) documentation.
• Effective care coordination and case management capabilities.
• Telehealth assessment and follow-up experience.
• Passion for educating patients and caregivers.
