As a Nurse Practitioner in Hospice Care, you will play a crucial role in providing palliative care, symptom management, and support for our hospice patients as requested by their referring physician.

Key Responsibilities:
• Conduct face-to-face visits with hospice patients within required time frames, reporting clinical findings to the Hospice Medical Director.
• Consult with the Hospice Medical Director as necessary and collaborate with primary physicians on recommended services.
• Develop and coordinate individualized care plans, documenting patient issues, goals, interventions, and responses to care.
• Work collaboratively with patients, families, attending physicians, and other members of the Interdisciplinary Group (IDG) to ensure comprehensive care.
• Educate and supervise patients and their families in self-care techniques as appropriate.
• Maintain thorough and accurate clinical notes on patients' conditions, promptly reporting any concerns to the Administrator and Hospice Medical Director.

Required Qualifications:
• Active unrestricted license in your state of residence.
• Current certification as a Clinical Nurse Specialist or Nurse Practitioner.
• Certification by a national body such as ANCC, NBCHPN, or AANP.
• Current CPR certification.
• 5+ years of nursing experience.
• Possess a valid driver’s license and reliable transportation.

Preferred Qualifications:
• Experience collaborating within an interdisciplinary team.
• Background in clinical nursing care in areas such as oncology, hospice, geriatrics, or intensive care.
• Palliative Care ARNP certification.

The compensation for this role typically ranges from $104,500 to $156,000, with total cash compensation including base pay and possible bonuses based on factors like local labor markets, education, and experience.
