Job Description
We are seeking an experienced LPN Nurse Navigator to provide dedicated patient support and care coordination.
Key Responsibilities Include
• Making up to 20 outbound calls daily to over 300 patients per month
• Conducting assessments within LPN scope to identify patient needs
• Developing and collaborating on personalized care plans
• Serving as a communication bridge between patients, families, and providers
• Educating patients on conditions, treatments, and preventive care
• Tracking progress and adjusting care plans accordingly
• Documenting interactions across healthcare systems and tools
Qualifications And Skills
• Active LPN license with 2+ years of hands-on patient care experience
• Experience in patient care coordination or case management
• Strong clinical judgment and dedication to patient engagement
• A HIPAA-compliant home office with secure high-speed internet
• Reliability to work Monday–Friday, with no night or weekend shifts
Preferred Qualifications Include
• Care coordination or case management certification
• Experience with diverse patient populations
• Familiarity with EHR systems and healthcare software
