NP Provider - Hospice & Palliative Care (Per Diem)

Role: Nurse Practitioner (NP) providing specialist palliative care consultation services in various settings, including nursing homes, hospitals, and community environments.

Responsibilities

• Implement and deliver palliative care consultations, focusing on symptom management, communication, goal setting, and hospice eligibility review.
• Develop care plans aimed at reducing illness burden for patients and families.
• Perform initial and follow-up visits, including documentation and treatment recommendations.
• Maintain effective communication with referral sources and team members.
• Participate in quality initiatives, staff education, and hospice operations, including on-call duties.
• Adhere to best practices in history-taking, examination, and care planning.
• Perform clinical and administrative tasks within scope of practice.

Qualifications & Skills

• Master’s degree in Nursing and graduation from an approved Nurse Practitioner program.
• NY State RN licensure and certification as an Adult, Family, Gerontology, or Palliative Care Nurse Practitioner.
• Preferred: at least 1-year clinical experience, hospice/palliative care expertise.
• Strong communication, teaching, organizational, and critical thinking skills.
• Ability to work independently, problem-solve, and collaborate within an interdisciplinary team.
• Proficiency in Microsoft Word and Excel; valid NYS driver’s license preferred.
• Flexibility to work varied hours and travel within the assigned area.

Additional Requirements: CPR-BLS certification strongly encouraged; hospice & palliative care certification preferred within two years of hire.
