Job Description
Role Summary
The LPN provides patient-centered care under the supervision of a Registered Nurse or Nurse Manager, following the LPN scope of practice. Responsibilities include assessing patients, administering medications, assisting with treatments, preparing patients for exams, and documenting care in medical records. The role involves coordinating services with the healthcare team, educating patients and families, and participating in staff training and continued education. LPNs may serve as preceptors for new staff and assist with special projects. They are expected to deliver culturally sensitive, relationship-based care tailored to patient needs, ensuring safety and comfort.
Key Duties
Perform routine nursing procedures and manage patient needs per physician orders
Access ports, administer medications, and perform sterile techniques
Coordinate care, communicate changes, and update care plans with RNs and physicians
Educate patients and caregivers on self-care using teach-back methods
Complete accurate documentation and adhere to regulatory guidelines
Participate in staff education, orientation, and professional development
Qualifications & Skills
High School diploma or GED, with necessary nursing licenses and certifications (LPN, BLS)
Preferred healthcare, oncology, outpatient experience
Strong clinical, assessment, organizational, and communication skills
Proficient in computer use, with reliable transportation and scheduling flexibility
