Licensed Practical Nurse - Hospice

We are seeking an Licensed Practical Nurse (LPN) to provide compassionate, patient-centered hospice care.

Key responsibilities include

• delivering direct patient care and coordinating services with the healthcare team
• completing accurate documentation in compliance with policies and regulations
• following personalized care plans developed by Registered Nurses
• accepting and transcribing physician orders
• sharing clinical progress notes with team members
• participating in interdisciplinary team meetings

Qualifications and skills include

• successful completion of nursing education from an approved school
• current LPN licensure and at least one year of clinical experience, preferably in medical/surgical settings
• ability to independently organize work and deliver effective care
• excellent communication skills (oral and written)
• CPR certification
