As part of MJHS Home Care, you will play a crucial role in delivering advanced clinical services and emotional support to patients recovering from illness, injury, or surgery in the comfort of their own homes. We are looking for a compassionate Licensed Practical Nurse (LPN) who possesses strong clinical skills and decision-making capabilities to oversee and facilitate patient recovery.
Your responsibilities will include:
• Performing patient revisits based on skilled need under the supervision of a Coordinator of Care RN.
• Implementing care plans and evaluating patient treatment outcomes.
• Documenting visits electronically within 24-48 hours.
• Observing, measuring, recording, and reporting clinical data related to patients' health status under RN direction.
• Communicating and documenting any signs of decline in a patient’s health as per RN guidelines.
• Reconciling and administering prescribed medications accurately.
• Executing nursing treatments as prescribed, such as urinary catheterization and sterile dressing changes.
Requirements:
• Valid NY State LPN License is required.
• Minimum of one year of Med/surg experience in a hospital or long-term care facility is necessary.
• Must possess a valid NY State Driver's License and have access to a car.
• Strong computer skills are essential.
