Job Description
Join our innovative Hospice and Palliative Care programs, which offer a comprehensive range of services for patients and families facing life-limiting illnesses and end-of-life care challenges.
As a Hospice Care Coordinator, you will play a vital role in assessing and developing care plans for patients referred to our Hospice services. You will facilitate the admission process for patients as they transition from the hospital to a Hospice program tailored to their unique needs and those of their family members.
In this position, you will also be a key member of our business development and marketing team, working to foster strong relationships with physicians, social workers, case managers, and discharge planners within the medical center.
Qualifications:
• Currently licensed as a Registered Professional Nurse in New York State.
• Bachelor's Degree preferred.
• At least two years of experience in Hospice nursing with strong physical assessment and interviewing skills.
• Client-service focused with exceptional written and verbal communication abilities.
• Advanced critical thinking and problem-solving skills, along with the capability to work independently.
• Bilingual in Spanish/English required in specific locations.
