Role: In-Home Nurse Practitioner or Physician Assistant

Responsibilities

• Conduct comprehensive health evaluations for Medicare and vulnerable populations in patients' homes
• Perform detailed health risk assessments and review medications
• Identify and address care gaps, educate patients on chronic disease management and prevention
• Utilize technology and data tools to document encounters and support value-based care
• Collaborate with remote clinical teams to ensure quality patient outcomes

Qualifications & Skills

• Licensed Nurse Practitioner or Physician Assistant
• Strong assessment, communication, and patient education skills
• Ability to work independently with flexibility
• Comfort with technology and data documentation
