Hospice Registered Nurse (RN)

Transitions Care is seeking a compassionate Registered Nurse to provide hospice care in home and facility settings. In this role, you will deliver skilled nursing focused on symptom management, comfort, and quality of life for patients with serious or terminal illness. You will complete assessments, create and update care plans, educate patients and families, and collaborate closely with an interdisciplinary team. Responsibilities
• Provide skilled hospice nursing care in patient homes and facilities, focusing on comfort and symptom management
• Complete comprehensive nursing assessments and develop individualized care plans
• Administer medications, treatments, and interventions per hospice and palliative care protocols
• Educate patients and families on disease process, medications, and end-of-life care options
• Coordinate with physicians, social workers, chaplains, CNAs, and other interdisciplinary team members
• Monitor patient condition, update care plans, and document care accurately in the EMRParticipate in care conferences and communicate changes in condition to the care team promptly
• Support patients and families emotionally, providing guidance and resources for advance care planning
• Ensure compliance with hospice regulations, policies, and best-practice standards
• Participate in on-call rotation and respond to urgent patient needs as assigned
