Compassionate Registered Nurse for Hospice Care

Transitions Care is searching for a dedicated and compassionate Registered Nurse to deliver exceptional hospice care in both home and facility settings. As a Hospice RN, you will play a vital role in enhancing the quality of life for patients facing serious or terminal illnesses. Your responsibilities will include providing skilled nursing focused on comfort and symptom management, while making a significant impact on patients and their families during a critical time. Key Responsibilities
• Deliver skilled hospice nursing care in patient homes and facilities, prioritizing comfort and effective symptom management.
• Conduct comprehensive nursing assessments and develop personalized care plans tailored to each patient's needs.
• Administer medications, treatments, and interventions according to hospice and palliative care protocols.
• Educate patients and families about disease processes, medications, and end-of-life care options.
• Collaborate closely with physicians, social workers, chaplains, CNAs, and other interdisciplinary team members to ensure cohesive care.
• Continuously monitor patient conditions, update care plans, and accurately document care in the EMR.
• Actively participate in care conferences and communicate any changes in patient condition to the care team promptly.
• Provide emotional support to patients and families, helping them navigate decisions and offering resources for advance care planning.
• Ensure adherence to hospice regulations, policies, and best practices to maintain high-quality care.
• Participate in the on-call rotation and respond to urgent patient needs as required.

Essential Skills
