Key Responsibilities:
• Deliver skilled nursing care in various settings, including homes and facilities.
• Conduct comprehensive patient assessments and monitor symptoms closely.
• Develop and continually update personalized plans of care tailored to individual needs.
• Administer medications and effectively manage pain and other symptoms.
• Educate patients and families about the disease process and comfort care options.
• Coordinate care seamlessly with physicians, social workers, aides, and chaplains.
• Document assessments, interventions, and outcomes accurately and in a timely manner.
• Participate in on-call rotation, responding promptly to urgent patient needs.
• Provide emotional support to patients and families during this critical time.
• Ensure that all care aligns with hospice regulations and organizational policies.

Required Skills:
• Expertise in hospice nursing.
• Proficient in palliative care practices.
• Strong knowledge of pain and symptom management.
• Skilled in IV therapy and medication administration.
• Ability to educate patients and families effectively.
• Experience in care planning and coordination.
• Familiarity with electronic medical record (EMR) documentation.

• Understanding of regulatory compliance in hospice care.
