Your Role as a Hospice Registered Nurse:
• Serve as the primary contact for patients and families, guiding them through their hospice journey with care and compassion.

• Provide skilled, hands-on nursing care based on tailored Plans of Care, delivering support in patients' homes or wherever they feel most comfortable.

• Conduct initial and ongoing assessments to closely monitor patient conditions and adjust care as needed.

• Collaborate closely with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, and chaplains—to develop personalized care plans.

• Offer emotional, physical, and spiritual support to both patients and their loved ones, including providing education and bereavement guidance.

• Educate caregivers on how to safely care for their loved ones, ensuring they feel confident and supported.

• Communicate crucial updates regarding patient conditions to the care team for timely interventions.

• Maintain clear and accurate clinical documentation following compliance standards and infection control protocols.

• Engage in quality improvement and scheduling initiatives to ensure that care remains consistent, compassionate, and efficient.

• Assist patients and families in navigating essential topics related to hospice care, medication administration, symptom management, and end-of-life processes.

Key Responsibilities:
• Conduct comprehensive patient assessments and provide education to patients and families.
• Deliver skilled nursing interventions aligned with patients' goals of care.
• Develop individualized care plans and monitor progress.
• Coordinate care with interdisciplinary teams and consult with medical professionals as necessary.
• Educate patients and caregivers about self-care techniques and end-of-life care considerations.
• Document all clinical findings and patient communications accurately and in a timely manner.
• Participate in on-call and patient support activities, providing responsive care at all times.
Essential Qualifications:
• Education: Must be a Registered Nurse.
• Experience: At least one year of RN experience in acute care, oncology, long-term care, hospice, or home health preferred.
• Licenses/Certifications: Current nursing license, CPR Certification, valid driver’s license, and automobile insurance.
