Are you a dedicated Registered Nurse looking to transform the lives of patients facing life-limiting illnesses? At Gentiva, we provide individualized hospice and palliative care, supporting both patients and their families with dignity and compassion.
What You'll Do as a Hospice RN / RN Case Manager:
• Serve as the primary point of contact for patients and families, guiding them through their hospice journey with expertise and empathy.
• Deliver and document skilled nursing care based on each patient's personalized care plan, whether in their home, assisted living, or another setting.
• Conduct initial and ongoing assessments to monitor patient conditions and adjust care as necessary.
• Collaborate with an interdisciplinary team including physicians, LPNs, CNAs, social workers, and chaplains to develop and refine care plans tailored to patient needs.
• Provide emotional, physical, and spiritual support to patients and their loved ones, offering education and guidance through the grieving process.
• Empower caregivers with training to ensure they can confidently provide safe care to their loved ones.
• Communicate critical updates on patient conditions to the care team to facilitate timely interventions.
• Maintain accurate clinical documentation and adhere to infection control and compliance standards.
• Support organizational quality improvement and scheduling initiatives to ensure care is delivered consistently and compassionately.
• Help patients and families navigate complex topics such as medication administration, hospice philosophy, symptom management, and end-of-life expectations.
Essential Functions:
• Perform thorough patient assessments and provide education to patients and caregivers as required.
• Implement skilled nursing interventions and symptom management aligned with patient goals.
• Develop and update individualized care plans to meet the unique needs of each patient.
• Coordinate care with interdisciplinary teams, ensuring effective communication with physicians and nurse practitioners.
• Educate patients and caregivers about disease processes, self-care techniques, and end-of-life care.
• Document clinical findings, physician orders, and care plans in a timely manner.
• Participate in on-call services, delivering competent and responsive care.

About You:
To be successful in this role, you should possess:
• A current Registered Nurse license.
• A minimum of one (1) year RN experience, preferably in acute care, oncology, hospice, or home health settings.
• Strong knowledge of hospice care principles and the unique needs of terminally ill patients.
• Excellent communication skills and the ability to interact with diverse populations.
• A valid driver's license and current automobile insurance.
Compensation and Benefits:
