As a Hospice Registered Nurse Case Coordinator, you will take complete nursing responsibility for the delivery of care to hospice patients according to their Plan of Care. You will continuously assess and enhance your performance to improve care quality and productivity. You will collaborate with other professionals to develop and implement treatment strategies grounded in scientific nursing principles, ensuring the physical, psychosocial, emotional, and spiritual well-being of patients at the end of life.
Key Responsibilities:
• Conduct initial nursing evaluations to determine hospice service eligibility within forty-eight (48) hours of referral.
• Identify and assess the physical, psychosocial, and environmental needs of patients and families, with re-evaluation no less than every fifteen (15) days.
• Initiate and coordinate comprehensive plans of care.
• Document identified problems, established goals, interventions, and patient/family responses to care.
• Engage collaboratively with patients, families, attending physicians, and other members of the Interdisciplinary Group (IDG) to provide daily care.
• Guide and educate patients and families in self-care techniques as appropriate.
• Maintain thorough and accurate clinical notes regarding each patient's condition.
• Demonstrate sensitivity and competence in any home setting regardless of the client's background.

Required Qualifications:
• Valid and unrestricted RN licensure in the state where you are practicing.
• Current driver's license, valid vehicle insurance, and access to reliable transportation or public transit.
Preferred Qualifications:
• At least 1 year of clinical experience.
• Current CPR certification or willingness to obtain certification within 90 days of hire.
Your compensation will reflect several factors, including local market rates, education, experience, and certifications. Beyond a competitive salary ranging from $83,000 to $100,000 annually for full-time employment, we offer comprehensive benefits, incentive programs, equity stock purchase options, and 401k contributions, all subject to eligibility guidelines.
