Hospice Patient Care Nurse Manager

Heartland Hospice is seeking a Hospice Patient Care RN Manager to lead our interdisciplinary care team. In this role, you will oversee hospice nursing services, coordinate patient care plans, and ensure high-quality, compassionate end-of-life care in home and facility settings. Responsibilities include supervising RNs and LPNs, conducting patient assessments, managing caseloads, ensuring regulatory and documentation compliance, and collaborating with physicians, social workers, and chaplains. The ideal candidate is a strong clinical leader with hospice experience, excellent communication skills, and a passion for patient- and family-centered care.

Responsibilities
• Lead and supervise hospice nursing staff, including RNs and LPNs
• Oversee and coordinate individualized hospice patient care plans
• Conduct and review patient assessments and update care goals
• Ensure compliance with hospice regulations, policies, and documentation standards
• Collaborate with physicians and interdisciplinary team members on care delivery
• Monitor quality of care, patient outcomes, and satisfaction
• Manage staffing, scheduling, and caseload distribution for nursing team
• Provide clinical guidance, training, and performance feedback to staff
• Support patients and families with education on hospice services and care options
• Participate in on-call rotation and assist with complex clinical situations
