Job Description

MJHS Hospice and Palliative Care seeks a Nurse Practitioner to join a team of MDs and NPs delivering symptom management and goal-setting discussions across nursing homes, hospitals, and community settings in the Bronx, Upper Manhattan, and Isabella Nursing Home. The NP provider may work independently or with a treatment team, with visits following referrals.

Duties include consultation, care planning, staff education, on-call rotation, and coordinating multidisciplinary resources to ease patient
