As a Hospice Nurse Practitioner Specialist, you will offer expert consultation in palliative care, symptom management, and supportive care tailored to meet the diverse needs of Hospice patients, as guided by the referring physician.

Key Responsibilities:
• Conduct face-to-face visits with qualifying Hospice patients within established time frames and communicate clinical findings to the Hospice Medical Director.
• Consult and collaborate with the Hospice Medical Director, primary physicians, and other healthcare providers to enhance patient care.
• Develop and coordinate individualized care plans, documenting problems, goals, interventions, and patient/family responses effectively.
• Provide education and guidance to patients and families on self-care techniques when appropriate.
• Maintain detailed and accurate clinical notes documenting the patient's condition and communicate any critical changes to the Administrator and Hospice Medical Director.

Required Qualifications:
• Active, unrestricted license in your state of residence.
• Current certification as a Nurse Practitioner or Clinical Nurse Specialist in your state.
• Certification from a recognized national body such as ANCC, NBCHPN, or AANP.
• Current CPR certification.
• Minimum of 5 years of nursing experience.
• Valid driver's license, vehicle insurance, and access to reliable transportation.

Preferred Qualifications:
• Experience in clinical care in areas such as oncology, hospice, geriatrics, or intensive care.
• Palliative Care ARNP certification.
• Experience working collaboratively within an interdisciplinary team.

The compensation for this role typically ranges between $109,500 to $164,000. Base pay and bonuses are contingent upon local labor markets and factors such as education and experience, possibly increasing with productivity and performance.
