As a Hospice Nurse Practitioner, you will provide vital consultation in palliative care, symptom management, and supportive services tailored to meet the needs of our Hospice patients as referred by physicians.
• This is a PRN/Inconsistent home visit position for Hospice Care.

Your Responsibilities Will Include:
• Conducting face-to-face visits to qualify Hospice patients within set timeframes and reporting clinical findings to the Hospice Medical Director.
• Consulting with the Hospice Medical Director as needed, updating the primary physician on recommended services, and collaborating with other physicians as necessary.
• Initiating and coordinating individualized care plans, documenting issues, goals, interventions, and patient/family responses to care. Working closely with the patient/family, attending physicians, and the IDG team.
• Instructing and supervising the patient/family in self-care techniques as appropriate.
• Maintaining thorough and accurate clinical notes regarding each patient's condition and reporting any changes to the Administrator and Hospice Medical Director.

Required Qualifications:
• Unrestricted active license in your state of residence.
• Current certification as a Clinical Nurse Specialist or Nurse Practitioner in the state of practice.
• Current certification from a national body such as ANCC, NBCHPN, or AANP.
• Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
• Current CPR certification.
• 5+ years of nursing experience.

Preferred Qualifications:
• Experience with interdisciplinary teams.
• Clinical nursing experience in oncology, hospice, geriatrics, or intensive care.
• Palliative Care ARNP certification.

Compensation for this role typically ranges from $104,500 to $156,000. The total cash compensation includes base salary and bonuses, determined by factors including local labor markets, education, experience, and performance. We adhere to all minimum wage statutes as applicable.
