As a Hospice Nurse Practitioner, you will play a vital role in delivering specialized consultation in palliative care and symptom management tailored to the needs of hospice patients. This position is PRN, allowing you to provide care as needed with face-to-face visits.
Key Responsibilities:
• Conduct face-to-face visits for hospice patients within expected timeframes and share clinical findings with the Hospice Medical Director.
• Collaborate with the Hospice Medical Director and primary physician to align on patient care and recommendations.
• Develop, document, and coordinate comprehensive care plans, addressing goals and patient/family responses.
• Educate and support patients and families in self-care techniques when appropriate.
• Keep precise clinical notes and communicate any significant findings or needs to the Administrator and Hospice Medical Director.
Requirements:
• Active Nurse Practitioner license in West Virginia.
• Ability to travel across Grant, Hardy, Hampshire, and Pendleton counties as required.
Schedule:
• Standard business hours: Monday through Friday, 8:00 AM to 4:30 PM.
• Evening and weekend shifts are available.
Qualifications:
• Unrestricted Nurse Practitioner license and national certification (ANCC, NBCHPN, or AANP).
• A minimum of 5 years of nursing experience.
• Current driver’s license, vehicle insurance, and a reliable mode of transportation.
Preferred Experience:
• Current CPR certification or willingness to obtain within 90 days of hire.
• Experience working within interdisciplinary teams.
• Clinical background in oncology, hospice, geriatrics, or intensive care.
• Palliative Care ARNP certification.
