Job Description
Home Care Registered Nurse (RN)
We are seeking an experienced Home Care Registered Nurse to provide professional nursing services in patients' homes, promoting recovery and independence.
Responsibilities include: actively participating in interdisciplinary care planning, delivering personalized nursing care as per the plan, fostering positive outcomes through collaborative practices, and accurately documenting services to ensure compliance and support quality improvement.
Qualifications: successful completion of nursing education from an approved school, current RN licensure in the state, at least one year of clinical experience in acute or long-term care, and CPR certification. Home health experience is preferred but not required.
Skills: strong clinical nursing skills, effective communication, computer literacy, problem-solving, and critical thinking abilities. Knowledge of medical record standards and adult learning principles is essential.
