Heartland Hospice, part of ProMedica, is actively seeking a dedicated and compassionate Registered Nurse to join our team, providing exceptional hospice care to patients with life-limiting illnesses in both home and facility settings. As a vital member of our team, you will conduct thorough assessments, manage pain and symptoms, administer medications, and develop personalized care plans tailored to each patient’s needs. Your role will involve collaborating with an interdisciplinary team to educate and support both patients and families, ensuring every individual receives care with comfort, dignity, and respect. You will also be responsible for accurate documentation, participation in on-call rotations, and contributing to quality and safety initiatives within a mission-driven, patient-centered environment.

Key Responsibilities:
• Deliver compassionate hospice nursing care in home and facility settings
• Conduct thorough patient assessments and devise tailored care plans
• Effectively manage pain and symptoms; administer medications and treatments as needed
• Provide education and emotional support to patients and their families regarding end-of-life care
• Coordinate care seamlessly with physicians and interdisciplinary team members
• Document all assessments, interventions, and outcomes with precision
• Participate in on-call rotations to respond promptly to urgent needs

Essential Skills:
• Experience in hospice and palliative nursing
• Expertise in pain and symptom management
• Proficiency in IV and medication administration
