Your Role as a Hospice RN / RN Case Manager:
• Serve as the primary contact for patients and families, guiding them through the entire hospice journey.

• Administer and document skilled nursing care tailored to each patient's individualized plan, whether at home, in assisted living, or any comfortable setting.

• Conduct thorough initial and ongoing assessments to monitor patient conditions and modify care as necessary.

• Collaborate with an interdisciplinary team—physicians, LPNs, CNAs, social workers, chaplains, and others—to create and fine-tune customized care plans.

• Provide emotional, physical, and spiritual support to both patients and their loved ones, offering education, comfort, and bereavement guidance.

• Teach caregivers how to safely and confidently care for their loved ones, offering hands-on training and emotional support.

• Communicate critical updates regarding patient conditions to the care team, ensuring timely interventions.

• Maintain accurate and current clinical documentation while adhering to infection control and compliance standards.

• Contribute to the organization's quality improvement and scheduling initiatives to ensure care remains consistent, compassionate, and effective.

• Help families understand important topics such as medication administration, hospice philosophy, symptom management, and end-of-life processes.

Compensation for this role may vary based on multiple factors including location, experience, education, skills, licensure, certifications, and departmental equity. Our organization offers a comprehensive benefits and rewards package beyond just base pay.
Salary Range: $71,450 - $89,335/year
Key Responsibilities:
• Conduct in-depth assessments and reassessments including in-person and telephonic evaluations, providing education to patients, families, and caregivers as needed.

• Deliver skilled nursing interventions aligned with care goals and effective symptom management.

• Create and update individualized plans of care that identify patient needs, visit frequency, and potential risks.

• Coordinate care with interdisciplinary teams, consulting with physicians and nurse practitioners as necessary.

• Educate patients and caregivers on disease processes, self-care methods, end-of-life care, medications, and nutrition.

• Report any changes in patient conditions to the appropriate healthcare provider.

• Facilitate continuity of care by recognizing and supporting transitions within home health, palliative, and hospice referrals.

• Ensure timely documentation of clinical records including assessments, progress notes, and care plans.

• Foster effective teamwork with the interdisciplinary group and involve necessary team members in the care plan.

• Participate in on-call and patient support services as required, providing competent and responsive care.

Your Qualifications:
• Valid Registered Nurse (RN) license.

• At least one year of nursing experience in an acute care setting, oncology, long-term care, hospice, or home health; internships and clinical hours may be counted in some situations.

• Previous experience in hospice or palliative care is preferred.

Licenses & Certifications:
• Current nursing license in the state of employment.

• Current CPR certification.

• Valid driver's license and current automobile insurance.

Additional Skills:
• Deep understanding of the needs of terminally ill patients and families, with the ability to apply that knowledge effectively.

• Strong assessment skills, excellent communication, and documentation abilities.

• Knowledge of regulatory guidelines and community resources related to hospice care.

Work Environment:
• Work primarily in patients' homes or institutional settings with potential exposure to various health hazards.
