On Call Registered Nurse

Heartland Hospice is seeking an On Call Registered Nurse to provide compassionate hospice and palliative care to patients in home and facility settings. The RN triages after-hours calls, conducts urgent visits, assesses symptoms, administers medications, and implements care plans focused on comfort and dignity. Collaborating with an interdisciplinary team, the nurse educates and supports patients and families, documents in the EMR, coordinates with physicians, and ensures timely, high-quality end-of-life care. This role requires strong clinical judgment, communication skills, and the ability to work independently in the field.

Responsibilities
• Provide after-hours and on-call hospice nursing care in homes and facilities
• Triage calls, assess patient status, and determine urgency of visits
• Perform urgent visits, complete head-to-toe assessments, and manage symptoms
• Administer medications and treatments focused on comfort and pain control
• Implement and update individualized hospice care plans with the team
• Educate and support patients and families on end-of-life care and expectations
• Collaborate with physicians, social workers, chaplains, and other team members
• Document assessments, interventions, and outcomes accurately in the EMRCoordinate transitions of care and report significant changes to case managers
• Follow hospice policies, regulatory standards, and infection control guidelines
