Clinical Pharmacist - Hematology/Oncology

Role Overview:

This pharmacist position involves delivering safe, effective, and cost-efficient medication therapy, developing evidence-based care plans, and monitoring patient outcomes.

Key Responsibilities:

• Provide comprehensive pharmaceutical care, including medication assessment, safety checks, and adherence monitoring.
• Interpret and clarify physicians' medication orders, review medication profiles for interactions, and suggest formulary alternatives.
• Consult with healthcare professionals on drug therapy issues, review lab results, and provide drug information.
• Conduct patient discharge medication counseling and participate in medical rounds.
• Calculate dosages, verify prescriptions, and prepare sterile and compounded medications.
• Supervise pharmacy staff, technicians, students, and interns, ensuring accuracy and adherence to standards.
• Coordinate pharmacy activities, maintain supplies, and assist in policy updates.

Qualifications & Skills:

Requires a Bachelor's Degree in Pharmacy (Doctoral preferred). Strong communication, clinical judgment, and teamwork skills are essential.
