Job Description

Nurse Practitioner | Palliative Care

A growing palliative care organization is seeking an experienced Nurse Practitioner to provide compassionate, high-quality care to patients with serious illnesses and complex chronic conditions throughout Ocean County and surrounding areas.

This role offers the opportunity to work within a collaborative interdisciplinary team while making a meaningful impact on patients and families through comprehensive symptom management, advance care planning, and patient-centered care.

Responsibilities

Conduct comprehensive palliative care assessments and follow-up visits Manage patients with serious illness and complex chronic conditions Facilitate goals-of-care conversations and advance care planning discussions Develop and implement individualized care plans Collaborate with physicians, specialists, hospitals, skilled nursing facilities, and community providers Provide symptom management and patient/family education Document patient encounters accurately and in a timely manner Participate in interdisciplinary team meetings and care coordination activities

Qualifications

Active New Jersey Nurse Practitioner license National board certification and DEA registration Minimum 2 years of Nurse Practitioner experience preferred Experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or chronic disease management preferred Strong communication and interpersonal skills Ability to work independently while collaborating within a multidisciplinary team

Preferred

Hospice & Palliative Care certification (ACHPN) Experience with home-based care, telehealth, or value-based care models Bilingual skills are a plus
