Role Overview: As an Advanced Practice Nurse, you will play a crucial role in delivering advanced nursing care to patients and their families while serving as an invaluable resource for both staff and the interdisciplinary team.
Key Responsibilities:
• Provide specialized primary care and consultations, including prescriptive authority and on-call responsibilities in collaboration with a supervising physician.
• Coordinate and manage care for patients with complex conditions through comprehensive assessments and leading interdisciplinary team meetings.
• Conduct face-to-face patient visits and ensure accurate documentation according to company guidelines and procedures.
• Offer clinical education for internal and external parties, support staff as a resource, and participate in team meetings and quality improvement initiatives.
• Support performance improvement efforts and fulfill additional responsibilities to contribute to clinical excellence.

Required Qualifications:
• Unrestricted nursing license in your state of residence.
• Certification as a Clinical Nurse Specialist or Nurse Practitioner in your practicing state.
• Current certification by a recognized national organization such as ANCC or AANP.
• Possession of NPI Number, DEA with prescribing privileges, PECOS enrollment, CAQH enrollment, Medicare number, and a referable Medicaid number.
• Valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
• Current CPR certification.
• At least 1 year of experience as a Nurse Practitioner in chronic disease management, family care, or primary care.
The compensation for this role typically ranges from $104,500 to $156,000, comprising base salary and bonuses, influenced by various factors including local job markets, education, and experience. We comply with all applicable minimum wage laws. Additionally, we offer a comprehensive benefits package, incentive programs, equity stock purchase options, and 401k contributions, all subject to eligibility requirements.
