As the Palliative Care Nurse Practitioner, you will provide specialized care and support to patients and their families through palliative care consultations. This includes delivering quality care to those facing serious illness with a prognosis of one year or less who are not yet ready for hospice services.
Key Responsibilities:
• Utilize effective and ethical evidence-based palliative medicine practices to deliver compassionate patient care, addressing physical, emotional, spiritual, and psychosocial needs.
• Apply expert clinical knowledge to develop comprehensive and effective care plans tailored to individual patient needs.
• Conduct thorough assessments by collecting relevant information from medical records, family history, and caregivers to inform care decisions.
• Perform comprehensive assessments utilizing palliative care frameworks, interpret clinical findings, manage medication reconciliation, and establish actionable plans based on patient diagnoses.
• Encourage ACHPN certification after two years of employment or upon meeting experience requirements.

Required Qualifications:
• Active RN/APRN licensure in your state of practice.
• Qualification as a Nurse Practitioner/CNS with prescriptive authority.
• Current BCLS certification.
• Valid CPR certification.
• Minimum of 5 years of clinical nursing experience.
• A valid Driver's License and reliable transportation.
Preferred Qualifications:
• Advanced practice palliative care certification (ACHPN).
• At least 1 year of experience as an APRN in Hospice, Palliative Care, or Oncology.
Compensation for this specialty typically ranges from $105,758 to $158,638, which includes both base pay and bonus. This range is influenced by local labor markets, education, and experience, with potential for increase based on performance. In addition to your salary, we provide a comprehensive benefits package, recognition programs, stock purchase options, and a 401k contribution, subject to eligibility.
