Role Description The Adult Nurse Practitioner will provide comprehensive primary and palliative care to adult patients through home visits telemedicine in Frederick County, MD. Day-to-day responsibilities include conducting health assessments, diagnosing and managing acute and chronic conditions, prescribing medications, ordering and interpreting diagnostic tests, and developing individualized care plans. The practitioner will collaborate with practice providers, office staff, home health & home care agencies, and caregivers to coordinate care, support transitions, and address symptom management, advanced care planning, and patient education. Documentation of encounters, adherence to evidence-based guidelines, and participation in quality improvement activities are integral parts of the role. This is a contract, hybrid position based in Frederick, MD, with a combination of in-person patient visits and some work-from-home for documentation, telehealth follow-ups, and care coordination.
Qualifications
• Active Nurse Practitioner license with independent practice in Maryland and national board certification as an Adult or Adult-Gerontology Nurse Practitioner.
• Demonstrated experience in adult primary care and/or palliative care, ideally in home-based or community settings.
• Strong clinical assessment, diagnostic, and treatment planning skills, including competence in managing chronic and complex conditions.
• Proficiency in electronic health records (EHR), telehealth platforms, and accurate, timely clinical documentation. athena experience is a plus.
• Excellent communication, counseling, and interpersonal skills for working with patients, families, and interdisciplinary teams.
• Commitment to inclusive, equitable, and trauma-informed care, with sensitivity to diverse cultural, socioeconomic, and gender identities.
• Ability to work independently in the field, manage schedules, and reliably travel to patient homes in the Frederick, MD area. Must live close enough to Frederick County to commute to all areas of the county.
• Master’s or Doctoral degree in Nursing from an accredited program; current CPR/BLS certification required.
• Experience with advanced care planning, end-of-life discussions, and symptom management is highly beneficial.
• Experience treating patients with substance use disorder is a plus.
