Join us as an On-Call Nurse, where you will have the opportunity to make a significant difference in the lives of patients and families. You will be entrusted to provide care after hours during urgent times, becoming a trusted face in homes filled with uncertainty. For many of our team members, this work is more than a career; it’s a calling.

Schedule: Saturdays and Sundays, 8:00 am - 8:30 pm

Your Responsibilities Will Include:
• Assessing the physical, psychosocial, and spiritual needs of patients and families (RN only).
• Collecting data related to the patient and family’s needs (LPN).
• Providing intermittent skilled nursing services under physician direction.
• Monitoring and evaluating symptoms, helping manage pain and other concerns.
• Acting as a liaison to the patient’s physician, sharing pertinent information.
• Offering emotional support to patients and families.
• Referring families to additional services as needed.
• Educating patients and families on care in a home setting.
• Making visits to residences at the time of death.
• Completing and returning all required documentation timely.
• Communicating and respecting patient confidentiality and dignity.
• Participating in team meetings and training sessions as necessary.
• Other duties as assigned.

What You Need to Succeed:
• High School Diploma or equivalent required.
• Graduation from an accredited nursing program; a Bachelor's degree is preferred.
• Current RN/LPN license in Ohio.
• Preferred experience in pain and symptom management, oncology, or mental health.
• Effective communication skills, both oral and written.
• Able to work independently and as part of a collaborative team.
• Proven knowledge and skills for providing care to all age groups.
• Ability to excel in stressful and rapidly changing conditions.
• Willingness to assess patients in varying residential and public environments.
• Proficiency in Microsoft Office, especially Word and Excel.
• Valid driver’s license and a good driving record.

Physical Requirements:
• Capability to stand and walk for extended periods.
• Manual dexterity and coordination for nursing care and equipment use.
• The role may require lifting up to 75 lbs. and frequent lifting of up to 50 lbs. with safe transfer techniques.
• Ability to stoop, squat, or bend regularly.
• Normal vision and hearing to communicate effectively with patients and staff.
• Willingness to face environmental factors and potential exposure to health hazards.
