Virtual Oncology Nurse Navigator - Remote Care Champion

Daymark Health is seeking an Oncology Nurse Navigator to join our integrated care team. You will provide telehealth-based triage, care coordination, and patient education for oncology patients, collaborating with Medical Directors, NPs, Social Workers, and Enrollment Specialists.

You will support patients through the cancer care journey, from screening to survivorship, delivering compassionate, patient-centered care over phone and virtual platforms. BLS and RI licensure or eligibility required.
