Job Title: Oncology Social Worker (LCSW-C)
Agency: B4Health IntraStaff - Amare
Shift: Day Shift (8:00 AM – 4:30 PM)
Guaranteed Hours: 40 Hours/Week
Requirements:
• Active Maryland Social Work License at the LCSW-C level required
• Equivalent advanced clinical social work license from another state may be considered
• Maryland licensure application must be in process prior to clearance
• Master's Degree in Social Work (MSW) required
• Hospital Social Work experience required
• EPIC experience required
• Experience conducting biopsychosocial assessments
• Experience with care coordination and discharge planning
• Knowledge of community, state, and federal resources
• Understanding of health insurance and financial implications of chronic illness
• Crisis intervention and suicide assessment experience
• Short-term counseling experience
• Experience facilitating family meetings
• End-of-life and advance care planning experience
Preferred Qualifications:
• Oncology Social Work experience highly preferred
• Palliative Care experience preferred
• Hospice Care experience preferred
• Experience in specialty healthcare practice settings
Job Duties:
• Complete comprehensive biopsychosocial assessments
• Develop individualized care plans and interventions
• Coordinate patient care and discharge planning
• Connect patients and families with community resources
• Provide crisis counseling and emotional support
• Conduct suicide risk assessments and short-term counseling
• Facilitate family meetings and care discussions
• Support patients and families through serious illness, terminal diagnoses, grief, and loss
• Assist with advance care planning and end-of-life discussions
• Utilize EPIC and telehealth platforms for documentation and patient care
Schedule Details:
• Day Shift: 8:00 AM – 4:30 PM
• Monday through Friday schedule
