This role involves providing case management and direct patient care in a home setting, specifically supporting the Monroe County team. Candidates must have Oasis experience and will work within a team to administer comprehensive physical therapy modalities.

Key Responsibilities of the Physical Therapist


- Provide case management and direct patient care within a team environment
- Administer physical therapy modalities in home settings
- Assist physicians in evaluating levels of function and developing care plans
- Prepare clinical and progress notes

Requirements of the Physical Therapist


- B.S., M.S., or D.P.T. degree in Physical Therapy
- Current New York State License in Physical Therapy
- BLS certification (AHA only)
- Minimum of one year experience in Long Term Care/Home Care
- Recent Home Care experience (within the last year)
- Oasis experience is mandatory
