Facility: Pathways Home Health and Hospice
Location: 585 North Mary Avenue, Sunnyvale, CA 94085 (Territory includes San Francisco, San Mateo, and Santa Clara Counties)
Shift: Days: Sunday – Thursday (10am – 7pm / 11am – 8pm) OR Tuesday – Saturday (10am – 7pm / 11am – 8pm); flexible schedule based on patient availability
Hours per week: 40 Hours/week
Contract Length: 13 Weeks

Pay Package:

• Weekly Gross: $2,475.00

• W2 Rate /Hr: $25.00(1-8 hrs)

• W2 Rate /Hr: $37.50(9-12 hrs)

• Housing and Meals Stipend: $1,475.00

Additional Benefits:

• Paid orientation

• Sick leave - 13 hrs

• Mileage Reimbursement: $0.76/Mile
