Job Requirements
• Must have professional license and/or certification as required for the position with primary source verification within 30 days of start date
• MGB Badge Photo required (color photo taken in past 6 months, plain background, no eyeglasses)
• MGB Radiation Badge Form required
• Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or proof of immunity via blood test
• Documentation of TB skin test screening within 3 months of start date or negative QuantiFERON TB Test or T-spot within 3 months of start date
• Physician Health Statement signed by MD/DO/NP/PA within 365 days of start date
• MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break
• MGB Massachusetts CORI check within 30 days prior to start date and updated if >6 month break
• MGB Resume including past 7 years of work experience
• Two references within 4 years of submission, minimum one supervisor or above in skillset
• MGB Skills Checklist completed within 1 year of submission
• MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions
• MGB Education Verification through background check company
• MGB SSN Trace with addresses/aliases for past 7 years
• MGB National Sex Offender Check within 30 days of start and updated annually
• MGB Exclusion Checks within 30 days of start and annually

Additional Details
• MGB Badge Photo must be a color photo taken in past 6 months with plain background and no eyeglasses
• Fit Test (N95) required within 365 days to start date and updated annually; no onsite fit testing available
• Respirator Medical Clearance Attestation required if fit test is required
• Hepatitis B vaccination strongly preferred
• Tdap vaccine preferred within 10 years and updated upon expiration
• Positive PPD requires negative chest x-ray documentation
• MGB Name Change Documentation required if credential documentation does not match current legal name
• MGB TB Symptom Survey required if positive PPD, completed prior to start and updated annually
• MGB Candidate Submission Coversheet required for submission to initial assignment and new facility
