• Provide hospice nursing care to patients in a home health setting
• Coordinate patient care plans in collaboration with families and healthcare teams
• Document patient assessments, interventions, and progress accurately
• Support patients and families through end-of-life care processes
• Ensure compliance with hospice regulations and standards
