Job Summary
We are seeking an experienced Hospice Registered Nurse (RN) to provide compassionate, patient-centered care to hospice patients and their families. The ideal candidate will have hands-on experience working within a home hospice agency/organization and strong case management skills.
Key Responsibilities
• Perform comprehensive patient assessments and determine appropriateness for hospice care.

• Develop, implement, and evaluate individualized plans of care for patients and families.

• Manage assigned hospice cases and prioritize patient needs.

• Conduct admission, re-visit, transfer, and discharge assessments and complete documentation within required timeframes.

• Provide age-appropriate patient and family education and document teaching provided.

• Coordinate care with the interdisciplinary team (IDT) and participate in biweekly IDT meetings.

• Perform medication reconciliation and maintain current medication orders.

• Provide HHA supervision according to established standards.

• Ensure appropriate utilization of hospice services and demonstrate knowledge of insurance reimbursement guidelines.

• Maintain accurate EMR documentation and complete required data transfers.

• Follow infection-control and patient-safety standards.

• Participate in Performance Improvement (PI) activities, staff meetings, and quality initiatives.

• Assist with orientation and mentorship of new employees.

• Cross-train and support additional agency needs as assigned.

Required Qualifications
• 2 years of RN experience in a home hospice environment — REQUIRED

• Must have experience working for a hospice agency/organization.

• Experience with EMR systems.

• Strong critical thinking, communication, and case-management skills.

• Associate's Degree in Nursing (ASN) OR nursing diploma with a valid NJ RN license.

• Current New Jersey RN License or Compact state license Required

• Valid Driver's License.

• Current BLS certification.

• English proficiency: Read, Write, Speak.

Preferred Qualifications
• 3 years of RN experience.
