We are in search of a Hospice RN - Regional Support who will play a crucial role in providing direct care to patients and essential support to their families. This dynamic position means you will also be a floating clinician, temporarily assigned to different hospice branches based on staffing needs. In this role, you will:
• Deliver exceptional, compassionate nursing care to hospice patients.
• Manage and implement tailored care plans for each patient.
• Support and lead interdisciplinary team members, including LPNs, CNAs, and HHAs.
• Evaluate and address the physical, emotional, spiritual, and environmental needs of patients.
• Ensure accurate documentation of all care provided in compliance with hospice protocols.
• Educate families and caregivers on care instructions, medications, and safety measures.
• Participate in the on-call rotation and provide necessary after-hours care.
• Collaborate effectively with physicians, referral sources, and your care team.
• Assist with referrals, regulatory documentation, and payer compliance.
• Travel to various Gentiva hospice locations as needed to fill urgent care gaps.

Schedule & Travel:

This position typically follows a Monday to Friday schedule, with occasional on-call duties. Travel will be required and may include:
• Overnight travel within a 100-mile radius.
• Frequent overnight travel to multiple states.
• Regional travel, including overnight stays based on assignment needs.

A company fleet car will be provided for eligible employees.

Qualifications - What You Will Bring:
• An active RN license in your state.
• A minimum of 1 year of RN experience in hospice, home health, acute care, long-term care, oncology, or similar high-acuity settings.
• Current CPR certification.
• Valid driver's license, auto insurance, and dependable transportation.

Preferred Experience (Not Required):
• Previous experience in hospice and end-of-life care.
• Strong skills in clinical assessments and compassionate communication.
• Exceptional critical thinking, adaptability, and problem-solving abilities.
• Comfortable in varied environments with changing needs.
• Proficient in documentation and electronic health records.
• Composed and supportive during crises, skilled at engaging families.
