Job Description
Location: Northern
Mohawk Valley Health System seeks a Registered Nurse Navigator to be the primary contact for cancer patients and families, coordinating care from pre-diagnosis through all treatment phases and survivorship, and helping overcome barriers to access.
The RN will educate patients and families, coordinate appointments, and collaborate with multidisciplinary teams, social work, and cancer registry to support planning and outcomes, including community outreach and education.
