Artemis Healthcare Partners is seeking an experienced clinical leader to join our client's home health team!
Position Name: Territory Clinical Manager (TCM)
Location: San Diego, CA
Employment Type: Direct-Hire & Permanent
Setting: Home Health
Pay: $110,000-$135,000
Shift: Full-Time
Qualifications:
Required:
- Graduate of an accredited professional nursing program with an active, unrestricted Registered Nurse (RN) license in the state of California.
- Minimum of 3-5 years of professional RN experience.
- Current CPR certification, with the ability to maintain certification throughout employment.
- Valid driver's license, reliable insured vehicle, and ability to travel between assigned business and patient care locations.
- Strong understanding of clinical standards, regulatory requirements, and quality expectations within a healthcare environment.
Preferred:
- Previous leadership or management experience overseeing nursing services.
- Experience within home health, community-based care, outpatient, or clinic settings.
- Familiarity with OASIS documentation, quality assurance, performance improvement, and survey readiness.
Job Summary:
- Provide leadership and oversight for daily clinical and office operations across the assigned territory, ensuring staff have the support and resources needed to provide high-quality patient care.
- Oversee clinical service delivery and ensure care is provided in accordance with organizational policies, professional standards, and applicable state and federal regulations.
- Coordinate referrals and collaborate with interdisciplinary team members to support timely admissions, individualized plans of care, ongoing assessments, transfers, and discharges.
- Serve as a clinical resource and provide operational coverage for key team members as needed, including supporting territory leadership in the Administrator's absence.
- Monitor the quality and effectiveness of patient care through clinical review, documentation oversight, discharge review, and evaluation of patient outcomes.
- Review and finalize applicable discharge documentation and OASIS records while ensuring accuracy and regulatory compliance.
- Lead quality assurance and performance improvement efforts, including QAPI initiatives, audits, trend analysis, and development of corrective action plans.
- Investigate clinical incidents, identify patterns or opportunities for improvement, and partner with leadership to implement appropriate follow-up measures.
- Support accreditation and regulatory survey preparedness through ongoing quality reviews, audit activity, and corrective action monitoring.
- Oversee emergency preparedness activities for the territory, including plan activation, event monitoring, and required post-event reporting.
