Pay Range: $25.20 - $39.69 per hour

Summary

The Oncology Care Management Supervisor is responsible for managing the outpatient care management navigation services provided to oncology patients and families engaged with Rush University Medical Center’s Cancer Center. The supervisor oversees development and operational oversight of the oncology care management navigation program, manages oncology care management navigators, and maintains a part-time caseload of patients in need of care management navigation services.

The oncology care management navigation program assists outpatient oncology patients in accessing resources and overcoming barriers to support a successful cancer care journey, working with multi-disciplinary teams of medical providers and supportive team members (e.g., social workers, community health workers) to promote the effective utilization of services and coordination of care for patients within the oncology service line. The navigators serve as a liaison between patients, oncology care teams, the hospital, and the community in performing functions that meet the needs of patients during their cancer treatments.

This position is part of the Supportive Oncology department, a multidisciplinary team supporting the RUSH MD Anderson Cancer Center. The individual who holds this position will exemplify the Rush mission, vision and values and act in accordance with Rush policies and procedures.

Other information:

• Associates degree in health-related area of focus and 5 years as a navigator or care coordinator OR 8 years of experience in care management/lay navigation service functions
• Completion of ACS-Lion Training
• Completion of annual training relevant to oncology navigation/principal illness navigation
• 3 years of experience with the Electronic Health Record
• Experience with high risk or complex populations
• Knowledge of community resources for psychosocial issues and expertise in oncology care management navigation
• Proficiency with basic Microsoft Office programs
• Strong written, verbal, and analytical communication skills
• Strong active listening, critical thinking, judgment, and decision-making skills
• Ability to function independently and take initiative as needed

Preferred Job Qualifications

• Staff supervision or management experience
• Experience working with adult oncology patients
• Strong interpersonal and teamwork skills
• Strong organizational, planning, and implementation skills, including problem-solving and troubleshooting
• Bilingual with QBS Certification

Physical Demands

Role requires physical mobility throughout the medical center as well as extended periods of seated work.

Responsibilities

• Oversee outpatient care management navigation services provided in Rush MD Anderson Cancer Center
• Manage and supervisor all Levels of navigators in the Supportive Oncology department
• Provide individual and group coaching, training, case consultation, and supervision as needed to support care management navigators in meeting the needs of Cancer Center patients and families
• Oversee navigation team workflow, referral process, caseloads, assessment and documentation tools, and data tracking systems
• Assist in monitoring new referrals based on acuity and complexity of patient needs
• Lead recruitment and onboarding for new care management navigators
• Manage part-time caseload of oncology patients in need of ongoing navigation services, working directly with patients and families to address patient barriers to care through navigation and care management services
• Complete assessments to clarify patient needs and identify patients at risk for adverse health outcomes, and develop action plans and basic care planning for high-risk patients
• Problem-solve with patients, their health care team, and other providers as needed regarding barriers to care
• Assist patients in completing forms, following through on assigned tasks, and/or accessing offered resources
• Consult and collaborate with multiple providers, departments, and agencies
• Maintain comprehensive knowledge of multiple aspects of the program so as to provide coverage as needed for missing navigators
• Provide on-call availability during Monday-Friday business hours as needed to support patients, providers, and navigation team
• Maintain relationships with referring Rush providers through consistent and timely communication, and collaborate with providers and teammates as needed to meet patient needs
• Participate in meetings as assigned
• Document all correspondence in Epic electronic health records system
• Collaborate with Supportive Oncology’s Manager of Social Work Services to ensure that social workers and navigators are effectively working together to support the needs of Cancer Center patients and staff
• Promote the Rush Supportive Oncology program and refer patients to department services as appropriate.
