As a Social Worker (LCSW), you will coordinate social work services, including psychosocial assessments, counseling, resource information, and referrals for patients and families, not limited to the oncology population. In this role, you will work closely with patients and their loved ones to develop coping skills and provide support throughout the challenges of illness, hospitalization, treatment, and continuing care. You will serve as a patient safety ombudsman and advocate for transparency and disclosure in the event of medical errors.

A caregiver in this role works days from 8:00 a.m. – 5:00 p.m.

A caregiver who excels in this role will:

• Coordinate social work services, not limited to the oncology population, to provide psychosocial assessment, counseling, resource information and referrals to patients and families.
• Provide care for chronically ill patient population by applying knowledge of stages of human development, ethnic, cultural and spiritual influences.
• Demonstrate knowledge of oncology diseases and treatments including level of understanding, reactions and expectations of disease process.
• Complete psychosocial assessments and develop short and long-term care plans based on individual need in a timely manner.
• Perform ongoing assessment and continued monitoring of treatment goals based on adjustment to chronic illness and change in functional status.
• Consult with other medical disciplines and staff regarding bio-psychosocial, environmental, ethnic and cultural factors which affect adjustment to chronic illness and its treatment.
• Educate and develop clinical care planning with patients, families and multidisciplinary teams based upon mutually agreed goals to enhance, maintain and promote optimal psychosocial functioning throughout chronic illness treatment and its outcomes.
• Partner with outpatient team, insurance and billing specialists in implementing policy related to social work and outpatient issues.
• Assist with securing financial coverage for treatments and medications consistent with quality patient care.
• Analyze and facilitate referrals to community resources and governmental programs including local, national health and welfare agencies including expertise in accessing assistance based on psychosocial need.
• Provide crisis intervention expertise as necessary including utilization of high-risk screening criteria for case findings and outreach activities which may include mental health and chemical and ETOH dependency resources.
• Initiate multidisciplinary team conferences regarding complex cases including advocacy for and protection of patient's dignity, confidentiality and right and access to care.
• Provide effective bereavement support, therapeutic counseling and assistance with referrals for Palliative Disease Management, Hospice and end of life issues.

Minimum qualifications for the ideal future caregiver include:

• Master’s Degree in Social Work
• Licensed Clinical Social Work (LCSW)
• Effective verbal, written and computer skills

Preferred qualifications for the ideal future caregiver include:

• Certification as Breast Patient Navigator in Breast Imaging and Cancer Care (CBPN-IC)
• Three to five years of clinical social work experience
• Two years of hospital Care Management experience
• Knowledge of InterQual guidelines
• Hospital or Hospice experience
• Strong behavioral health background

Physical Requirements:

• Essential Duties: This position requires knowledge of all aspects of patient care including assessing; planning, coordinating, monitoring and evaluating the patients progress through the entire continuum of care.
• Physical Duties: This position requires frequent walking, sitting, and standing. Frequent talking and listening when giving instructions and explanations. Frequent use of the phone; fax machines, and occasional use of the computer.
• Working Conditions: The majority of time the employee spends indoors in air-conditioned areas. Potential for exposure to blood and blood borne pathogens is significant. Clinical exposure to patients requires the use of personal protective equipment.

Personal Protective Equipment:

• This position requires minimal exposure to infectious wastes such as blood and bodily fluids that mandate the wearing of gloves, masks, gowns, and goggles as appropriate.
• Follows Standard Precautions using personal protective equipment as required for procedures.
