Social Worker - Hospital & Patient Services

Adventist Health is seeking a Social Services Practitioner to support whole-person care for patients and families in our faith-based, mission-driven environment.
In this role, you will perform psychosocial assessments, develop care and discharge plans, and provide counseling, crisis intervention, and resource navigation.
