RN Nurse Navigator

As an RN Nurse Navigator at Mercy, you will guide patients and families through the care continuum, coordinating appointments, tests, and treatments across specialties. Using clinical expertise and strong communication skills, you'll educate patients, advocate for their needs, and collaborate closely with physicians and interdisciplinary teams. You'll track patient progress, support care transitions, and promote safety, quality, and adherence to care plans while contributing to process improvement and mentoring peers in Mercy's compassionate, mission-driven environment.

Responsibilities
• Coordinate and navigate patients through the full continuum of care
• Conduct clinical assessments and monitor patient progress
• Educate patients and families on diagnoses, treatments, and resources
• Collaborate with physicians and interdisciplinary teams on care plans
• Arrange and track appointments, referrals, and diagnostic tests
• Support safe care transitions and discharge planning
• Document thoroughly in the EHR and track key patient metrics
• Identify barriers to care and advocate for patient needs
• Participate in quality improvement and care standardization initiatives
• Mentor peers and contribute to a collaborative, compassionate culture
