Job Description

Overview:

The primary responsibility of the RN Navigator is to assess patient care for optimal collaboration with Radiologists, Physicians, and Medical Consultants. Operated with minimal supervision, this position acts as a pivotal point for clinical and administrative correspondence, drawing on expertise to create, examine, and organize essential operational and clinical procedures. Consulting senior management for complex cases is a crucial element of this role.

Responsibilities:

Reviews patient instances, medical records, and ongoing treatment approaches to adeptly lead patients through their healthcare trajectories.

Formulates personalized care blueprints, in conjunction with a variety of healthcare providers and departments, in order to offer patients a well-organized and nurturing treatment approach.

Documents patient communications, care scheme evolutions, and guidance strategies to promote coherent and organized healthcare provision.

Employs expertise in healthcare networks, resources, and patient requirements to direct individuals as they navigate their medical path, securing access to essential services and aid.

Analyzes patient-centric navigational strategies and interventions for effectiveness and relevance.

Ensures ongoing, transparent communication with healthcare teams and patients to maintain clarity and continuity of care, serving as a valuable resource for enhancing patient navigation services within the organization, and at times, turns to senior management for advice on complex navigation challenges.

Undertakes supplementary duties as directed.

Qualifications:

EDUCATION:

Required:
• Associate's Degree in Nursing

Preferred:
• Bachelor's Degree in Nursing

EXPERIENCE:

Required:
• 5 years as a Registered Nurse with 1 year in a clinical specialty as defined by ANCC or national nursing organizations

LICENSURE/CERTIFICATION/REGISTRY/LISTING:REQUIRED

Required:

Registered Nurse license in North Carolina or a Compact state

BLS (CPR)-American Red Cross or AHA Healthcare Provider
