AuthoraCare Collective is currently seeking a Hospice RN Long-term Care. This position is full-Time, Monday - Friday 8:00am - 5:00pm. CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Job Summary

Manage the over-all plan of care for a case load of hospice patients in collaboration with other members of the interdisciplinary group (IDG). Assess and identify patient/caregiver problems, establish nursing goals, and interventions. Work with physician to establish medical plan of care for the patient. Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching.

Work closely with other members of the interdisciplinary group and communicates effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Document the outcome of care. Provides dietary counseling for patient care needs as indicated. Assume on call responsibilities as scheduled.

Requirements

Education/Experience:

• Graduate of an accredited School of Nursing
• Must be currently licensed as an RN in North Carolina and in good standings with NCBON
• One year of experience in an acute, hospice, home health setting, or long-term care setting.
• Bachelor's degree in nursing preferred.
• Two years of experience preferred.
• CHPN Certified preferred.
• CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
• Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

Key Responsibilities

Delivery of Patient Care

• Directs and coordinates the care and services provided to patients in accordance with the patient’s plan of care and based on all assessments.
• Responsible for continuous assessment of patient/family needs and implementation of interdisciplinary plan of care.
• Works with IDG to identify patient/family goals of care and establish and maintain the plan of care.
• Utilizes nursing process in planning and prioritizing patient care, including assessment and identification of patient/caregiver problems; identification and implementation of interventions; and evaluation of outcomes.
• Performs regular re-evaluation of the patient/family/caregiver situation to meet their needs and maintain continuity of care.
• Establishes therapeutic relationship with patient/caregiver, including providing emotional support and teaching about disease progression and symptom management.
• Recognizes influence of culture, ethics, and beliefs on clinical decision making.
• Makes home visits in accordance with Plan of Care or at patient's/caregiver's request.
• Communicates changes to the plan of care to patient/family and any facility staff involved.
• Delegates care to LPN in accordance to Nursing Practice Act providing indirect supervision to LPNs.
• Develops and oversees Hospice Aide plan of care, making supervisory visits to ensure plan of care is followed.
• Complies with infection control and exposure control policies.
• Works within scope of practice for RN according to NC Nurse Practice Act. Incorporates changes/advances in nursing practice into own practice.

Interdisciplinary Group Process

• Actively participates with the interdisciplinary group (IDG).
• Works effectively with other IDG members with a focus on collaboration in problem-solving patient needs and working with the patients’ plan of care.
• Provides effective report to the IDG based on regulatory requirements for patient care planning evaluation.
• Maintains effective relationships and partnerships with community healthcare providers.

Documentation

• Completes required documentation according to licensure and certification requirements and agency standards.
• Ensures that notes and patient records are complete, accurate, current and clear.

On-Call Responsibilities

• Assumes on-call responsibilities as scheduled.
• Makes, provides, and responds to calls in a professional manner.
• Triages calls and delegates and/or makes home visits based on assessment of problems and/or patient/caregiver request. Utilizes clinical resource.
