Job Summary:

We are seeking a compassionate and dedicated Registered Nurse (RN) Home Health Case Manager for our Home Health client in Portland, OR. In this role, you will coordinate and deliver high-quality, individualized care to patients in their homes. The RN Case Manager is responsible for assessing patient needs, developing care plans, and ensuring timely and effective nursing interventions in collaboration with interdisciplinary team members.
Key Responsibilities:
• Perform initial and ongoing assessments of patients in the home setting.
• Develop and implement individualized care plans based on patient assessment and physician orders.
• Serve as the primary clinical point of contact for assigned patients.
• Provide skilled nursing care including wound care, IV therapy, medication management, and patient education.
• Coordinate services with physicians, home health aides, therapists, and other healthcare professionals.
• Monitor and document patient progress and adjust care plans as necessary.
• Ensure accurate and timely documentation in the electronic medical record (EMR).
• Educate patients and families on disease processes, medications, nutrition, and safety measures.
• Comply with all regulatory requirements, agency policies, and best practice standards.
• Participate in case conferences, in-service training, and quality assurance activities.
Qualifications:
• Current, active RN license in Oregon.
• Minimum of 1 year Home Health experience.
• Post-acute care experience preferred.
• Knowledge of Medicare/Medicaid regulations and OASIS documentation.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and manage a caseload efficiently.
• Excellent communication, interpersonal, and organizational skills.
• Proficient in the use of EMR systems and mobile technology. Home Care Home Base or EPIC preferred.
• Valid driver’s license, reliable vehicle, and auto insurance.
