We are in search of a skilled and empathetic Hospice Clinical Liaison to join our team. This position requires you to efficiently and effectively manage intake and referral processes, catering to the needs of patients, families, referral sources, and our organization. You will play an essential role in fostering professional relationships, arranging admissions, and promoting awareness of hospice services.
Essential Responsibilities
• Respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare providers
• Determine patient eligibility for hospice services by reviewing medical records and conducting clinical evaluations
• Communicate with physicians and families to assess eligibility
• Inform patients and families about hospice care, including its purpose and benefits
• Complete necessary documentation and manage the intake and admission process
• Ensure all requirements - legal, clinical, and payer - are satisfied
• Act as a bridge between referral sources and our hospice care team
• Ensure seamless transitions, clear communication, and quick response to needs
• Conduct daily visits to hospitals and facilities
• Maintain visibility and accessibility with discharge planners, case managers, and physicians
• Build and sustain strong relationships with referral partners
• Identify new outreach opportunities to increase awareness of hospice services
About You:
Education
• Licensed Nurse with three to five years of hospice experience required
• Preferred: BSN with five or more years of hospice experience
Experience
• Minimum of three years of hospice experience necessary
• Previous work as a discharge planner, case manager, or educator is a plus
Licenses and Certifications
• Current RN license
• CPR certification
• Valid driver’s license and automobile insurance
Skills and Knowledge
• In-depth understanding of hospice care and patient family needs
• Knowledge of community resources and referral channels
• Strong skills in patient assessment and documentation
• Ability to supervise staff, coordinate services, and manage programs
• Effective communication and interpersonal skills with cultural sensitivity

• Flexibility in handling various tasks and shifting priorities
• Professionalism, teamwork, and tactfulness in interdisciplinary settings
• Ability to educate and influence referral sources, patients, and families
