Rehab Nurse Navigator for Orthopedic Trauma & Oncology

UofL Health is seeking a Rehab Nurse Navigator to coordinate a comprehensive rehab care continuum at the Frazier Rehab Institute. The role supports patients from pre-admission through post-discharge, providing education, documentation, and liaison across care teams.

A strong focus on improving patient outcomes and staff competency is emphasized. The ideal candidate has 2+ years in rehab nursing, ADN required with BSN preferred, and ARNP licensure not required.
