Inova Health Center 8081 - Fairfax East is looking for a dedicated Registered Nurse (RN) Patient Care Navigator - Structural Heart to join the team. This role will be Full-Time; Monday - Friday, No Holidays, No Weekends. Relocation assistance eligible.

Registered Nurse (RN) Patient Care Navigator - Structural Heart Job Responsibilities:

Coordinates a patient's plan of care which may include appointment scheduling, confirming patient's attendance, transportation, assisting with referral process, and answering patient questions.

Acts as a liaison, advocate and point of contact to patients in need of services such as screening, diagnostic treatment, clinical research participation, support services, and follow-up/aftercare plan by helping them access the best healthcare outcomes from doctors, hospitals and others who provide their required services.

Refers cases to the appropriate resources for assistance. Anticipates patient's needs and advocates for patient rights and appropriate utilization of services.

Conducts comprehensive assessment of the patient's health and psychosocial needs and develops a case management plan in collaboration with the patient and their support system.

Educates patient, family or caregiver(s), and members of the health care delivery team regarding treatment options, community resources, insurance benefits, self-care management, etc. to facilitate timely and informed decision-making throughout all phases of the continuum process.

Collaborates with multidisciplinary teams to address patient questions and responses about the care process and treatment options.

Develops/maintains reliable systems to document, track and monitor patient data.

Performs other duties as assigned.

Minimum Qualifications:

Education: Bachelors Degree in Nursing

Experience: 3 years of nursing experience

Certification: Licensed in the Commonwealth of Virginia as a Registered Nurse; Basic Life Support from the American Heart Association

Preferred Qualifications:

Experience: cardiac experience (CVICU/ICU, Cath lab, telemetry, etc.); familiarity with cardiac medications
